Ask the resident’s care team what form of transportation is appropriate. Identify one facility contact who can discuss the journey and direct clinical questions to the right professional.

Make the destination clear

An appointment, a hospital transfer and a move to another facility involve different arrival arrangements. Ask who expects the patient and whether the plan includes a return trip.

03 / Plan

Keep the family informed

Agree on who will receive timing updates. Ask the facility how personal belongings and necessary documents will be handled. Do not assume a family member can accompany the patient without discussing it first.
