Start with the sending hospital

Ask who is coordinating the transfer and who can explain the patient’s care requirements. The clinical discussion should connect the relevant professionals rather than depend on family members relaying medical instructions.

02 / Prepare

Clarify the receiving arrangement

Ask which destination is expecting the patient and who is responsible for the arrival handoff. A destination name alone does not establish that arrival arrangements are in place.

03 / Plan

Confirm timing and updates

Discuss how pickup timing will be communicated and whom to contact if the plan changes. A request begins the conversation; confirmation establishes the arrangement.
